What is the Community HealthChoices Program?

Community HealthChoices (CHC) is a free program for those who are eligible, funded through Medicaid and Medicare. This managed care program in Pennsylvania helps older people with disabilities get the care they need. It brings together healthcare services and long-term support to help individuals live independently. This program is overseen by the Office of Long-Term Living (OLTL) and aims to make it easier for people to receive the proper care, whether they need medical treatment or long-term services, to live comfortably at home.

Who is Eligible for Community HealthChoices?

You may qualify for CHC if you meet any of the following criteria:

  • You are 21 years old or older.
  • You are eligible for Medicare and Medicaid (or “dual eligible”).
  • You receive long-term services and support (LTSS) through Medicaid due to a physical disability or health condition, whether you are living at home or in a nursing facility.

If you meet these conditions, you must enroll in a Managed Care Organization (MCO) under CHC. This MCO will coordinate your care to ensure you receive the appropriate health services and support.

Once you are inralled in the program, we can deliver refrigerated, frozen, and shelf-stable meals directly to your doorstep in all 67 Pennsylvania counties, including Philadelphia, PA.
